Position works collaboratively with multidisciplinary health care team to facilitate plan of care for patients diagnosed with cancer. Assists in the development and implementation of cancer- related educational and outreach activities as well as monitoring program quality assurance. Requires oncology nursing experience, must have or obtain a WA RN license, BLS, and an Oncology Nursing Certification, or must obtain within six months of hire.

The Supervisor is responsible for monitoring clinical and operational activities of the medical oncology/infusion practices within the Swedish Cancer Institute at multiple campuses. The Supervisor will develop, implement, and maintain systems supporting facility operations and patient care. Within each role, the Supervisor provides direct patient care and collaborates, communicates and coordinates with physicians, interdisciplinary team members and patient families.

The successful candidate will hold OCN certification, posses an RN license from a National League for Nursing-accredited school of nursing in the State of Washington and possess a baccalaureate in Nursing. This position requires a current DEA number (or the ability to acquire one within 90 days of hire), a minimum three years of experience as a supervisor (including personnel supervision) and relevant work experience in Oncology Nursing.

Support Groups

Manage hire, train, and supervise lead facilitators and interns

      Program Evaluation:

      Supports agency efforts to evaluate and continuously improve programs.

Overlake Hospital's Cancer Center in Bellevue, WA is looking for a Nurse Navigator to provide clinical and emotional support to lung and prostate cancer patients.  The Navigator partners with patients, families, interdisciplinary team, and community resources to provide well-coordinated, timely, compassionate, and exemplary interdisciplinary care.  This involves, in part:

Job Requirements: 
BSN or an equivalent combination of education and experience is required.  Must also have current WA State Registered Nurse license and 4+ years clinical nursing experience, of which at least 2 years must be in oncology nursing.  Oncology Certified Nursing (OCN) certification preferred.  The position requires continuous walking/standing and the ability to lift, move or assist patients weighing up to 300+ pounds with help of other staff members.

About the Department 

This is a Unique career opportunity in Nursing.  This position focuses on a care coordination, assessment and the patient/cargiver education required to safely transition stem cell transplant patients to the various sites of care with emphasis on discharge planning.  In addition to direct patient care, this position supervises 3 RN’s and 2 Support Staff in this progarm.  This position is also responsible for utilization review on 2 units at the UWMC. 

 Responsibilities 

Clinical Responsibilities

Able to perform all responsibilities of Transition RN role. Clinically manages a patient case load.

Develop and implement a patient/family transition program that includes the discharge program.

Develop nursing standards for patient/family and caregivers transitioning through areas of care.

Collaborate with other nursing programs in the areas of education, clinical practice, leadership and research.

Develop nursing care planning and communication tools to facilitate the transition process.

Participate in quality improvement initiatives of patient/family care throughout transitions.

Collaborate with a multi-disciplinary team to ensure continuity of patient/family care throughout transition.

Serve as contact for insurance company throughout transplant process.

 

Function as primary contact for home infusion and skilled nursing facilities.

 

Resource for DME/infusion and diabetic monitoring equipment.

 

Utilization Review

Ensures utilization review occurs in a correct and timely manner for patients on 7NE and 8NE

Work closely with UWMC staff regarding UR policies and procedures.

Provide utilization review for transplant patients during the outpatient phase.

Supervision

Supervise the nurse Transition planners, the UR Team Coordinator and the Nursing PSR.

 

Performs all aspects of personnel management, including interviewing, hiring, performance evaluations, and compensation review.

Approves/disapproves staff time off and payroll.   Manages staff attendance and time off schedules.

Regularly conducts staff meetings.

Assists staff in prioritizing workloads when necessary

Develops and oversees orientation and training plan for new employees. 

Develops continuing education materials and processes.

Provides feedback to Nurse Manager regarding support staff performance 

 

Major Duties 

The Transition Nursing Supervisor is responsible for day-to-day operations, quality assurance and setting the standards for the nursing care of blood and marrow transplant patients during their transitions throughout the transplant program.  In addition, this position is responsible for the utilization review for inpatients admitted to the UWMC’s floors 7NE and 8NE.  This position provides direct patient care in addition to program development and supervisory responsibilities.

 

Independently responsible for the direct supervision of nurse Transition Planners, the Utilization Review Team Coordinator and Specialty RN Support PSR. This includes hiring, training, discipline, termination and performance evaluations.  Makes operational and procedural decision with input from the Transplant Clinic Nurse Manager.

 

This position collaborates with medical leaders, peers and colleagues at the Seattle Cancer Care Alliance (SCCA) as well as our partner institutions (FHCRC, UWMC and CHRMC) to assure the provision of high quality, patient and family-centered care in an environment that facilitates the conduct of clinical research.
